2014 Qualifying Information

Qualifying for county and district candidates, including write-in candidates, will be held in the Titusville Supervisor of Elections office from Noon, June 16, 2014 - Noon, June 20, 2014.

Beginning June 2, 2014, you may hand deliver or mail your qualifying paperwork to the Titusville Supervisor of Elections office to the address listed below. All qualifying paperwork must be completed and received by the Supervisor of Elections office no later than Noon, June 20, 2014.

Qualifying Requirements:

For qualifying purposes, each form requires your original signature.

Required Documents for County Commission Candidates:

• Appointment of Campaign Treasurer Designation of Depository for Candidates (DS-DE 9)
• Statement of Candidate (DS-DE 84, is not required to qualify but this form must be submitted within 10 days of filing a DS-DE 9)
• Oath of Candidate (DS-DE 24 for Partisan Candidates, DS-DE 24B for NPA Candidates)
• Full and Public Disclosure of Financial Interests (Form 6)
• Candidate Petition Certification or a campaign check for the payment of the qualifying fee

Required Documents for School Board Candidates:

• Appointment of Campaign Treasurer Designation of Depository for Candidates (DS-DE 9)
• Statement of Candidate (DS-DE 84 is not required to qualify but this form must be submitted within 10 days of filing a DS-DE 9)
• Oath of Candidate (DS-DE 25A or a DS-DE 24F for Write-in Candidates)
• Full and Public Disclosure of Financial Interests (Form 6)
• Candidate Petition Certification or a campaign check for the payment of the qualifying fee

Required Documents for Partisan Special District Candidates:

• Appointment of Campaign Treasurer Designation of Depository for Candidates (DS-DE 9)
• Statement of Candidate (DS-DE 84 is not required to qualify but this form must be submitted within 10 days of filing a DS-DE 9)
• Oath of Candidate (DS-DE 24 for Partisan Candidates, DS-DE 24B for NPA Candidates)
• Statement of Financial Interests (Form 1)
• Candidate Petition Certification or a check for the payment of the qualifying fee (Special District Candidates may pay with a campaign check or a personal check)

Required Documents for Nonpartisan Special District Candidates:

• Oath of Candidate (DS-DE 25 or a DS-DE 24A for Write-in Candidates)
• Statement of Financial Interests (Form 1)
• Candidate Petition Certification or a check for the payment of the qualifying fee (Special District Candidates may pay with a campaign check or a personal check)

Special District candidates who collect contributions and make expenditures other than the qualifying fee or petition verification fee are required to complete the following:

  • Appointment of Campaign Treasurer Designation of Depository for Candidates (DS-DE 9)
  • Statement of Candidate (DS-DE 84 is not required to qualify but this form must be submitted within 10 days of the candidate filing a DS-DE 9)

Important Reminders:

1. Please type or print your name the way you wish it to appear on the ballot under the “Oath of Candidate” section on the Candidate Oath form. It is important to remember, your name as it appears on the ballot cannot not be changed after the close of the qualifying period.

2. Your completed Candidate Oath form and the Form 6 require notarization. For your convenience, a notary will be available in the Titusville office during the qualifying period.

3. If you do not qualify by petition and choose to pay the qualifying fee, you must do so by Noon, June 20, 2014. It is important to remember that your qualifying fee cannot be returned to you unless you withdraw your candidacy with a signed notice before the close of the qualifying period.

4. In preparation for qualifying, please review the telephone number and email address you provided on your DS-DE 9 to ensure this information is up-to-date. If there is a problem with your qualifying paperwork, our office will make every effort to reach you using the contact information provided on this form.
